Immunization records

Vaccination records (also called immunization records) provide a history of all the vaccines you or your child have received.

Request health records

B.C. residents can access their health records online using Health Gateway. It is accessible by anyone 12 and older with a B.C. Services Card. 

Health Gateway only shows immunizations in the Provincial Immunization Registry; this includes immunizations given at public health clinics and pharmacies in B.C. It does not show immunizations given by other health care providers (i.e. family doctors/nurse practitioners).

If you are trying to find immunization records that are not in Health Gateway

  • Contact the provider that gave the vaccines.
  • Contact your community health nurse if the vaccines were given in a First Nations community.
  • If you have moved to BC from another province or country, try to get the records sent to you.

Child Health Passports

If your child was born in B.C., you should have received a Child Health Passport. The Child Health Passport is a passport-sized booklet for parents to use to record important health information about their child, including height and weight, illnesses, accidents, allergies, hospitalizations, and immunizations. The passport also provides safety tips and a space to record findings for hearing, dental, and vision checks.

Always keep a current immunization record for your child in a safe place. You may need the record if you register your child for daycare, summer camp, college or university and some travel.

Report your child’s immunization records to VCH

Vancouver Coastal Health needs to have an up-to-date record of your child’s immunizations if your child attends school or a licensed childcare facility so we can: 

  • Respond effectively to disease outbreaks in child-care facilities and schools.
  • Recommend immunizations your child may be missing.
  • Provide the best care for your child through our public health programs.
     

Children without vaccination records (or proof of immunity to disease) are considered unvaccinated and unprotected. Children who miss immunizations or have missing records may be asked to stay home if an infectious disease happens at their school or daycare. 

  • Report your child's immunization record before they start kindergarten.
  • Report every time your child receives a shot from a health-care provider, such as your family doctor.
  • Report your child's immunization record if you have just moved to the VCH region.

How to report your child’s immunization record to VCH

You must refer to your Child Health Passport or immunization records. You do not need to report immunizations given to your child at a VCH community health centre or school by a Public Health Nurse because those immunizations are automatically provided to you.
